All orders and money must be turned in NO LATER than May 3rd to the Yankee Candle folder at St. Viator.

Seller, please keep the pink copy of the order form for reference at delivery. Return the yellow and white copies with your order packet.

Please...keep the colored brochure as reference when delivering orders to your customers.

Be sure that the seller’s name and ALLIGATOR AQUATICS SWIM TEAM are printed at the top of your order form.

This is a triplicate form, please print clearly.
